The ceremony to become a fourth degree member of the Knights of Columbus
is called an Exemplification of the Fourth Degree.
The Exemplification of the Fourth Degree was held in Lisle, IL on:
One hundred and fifty eight Third Degree Knights joined the ranks of the Fourth Degree.
Mark your calendars, our next exemplification will be held:The degree will be held at the Wyndham Hotel in Lisle. More details will follow as we get closer to the degree date.

- Contact your councils fourth degree representative,
Grand Knight, or Financial Secretary for a Form 4 Application.
- Download the
Reservation Form
and the
Tri-Fold Flyer.
- Using the information in the flyer, fill out the Form 4 and
Reservation Form and submit to a Sir Knight that will sponsor you.
Make sure the information is clear and complete. The First Degree
Date and Membership number MUST be listed.
- The submitted form will be reviewed and voted on by the Assembly.
Once accepted and approved, the Assembly will contact you for any
outstanding fees.

- You must be a Third Degree member in good standing.
- You must have received your First Degree one year prior to
the Fourth Degree date.
- You must be sponsored by a Sir Knight, and approved for membership
by a Fourth Degree Assembly.
- Exemplifications are usually held only once a year.
| WAIVER NOTICE: | In some cases a waiver of six months may be granted for the one year requirement. The Navigator of the Assembly MUST request this waiver from the Master, in writing, at least 30 days prior to the Exemplification. |
The District helps offset some of the costs of the exemplification via ads in the
Program Ad Book. Each Council and Assembly is asked to support this effort.

The dress code for the exemplification for candidates and Sir Knights
will be a black tuxedo, white shirt
(NO FRENCH COLLARS, PLAIN LAYDOWN COLLAR ONLY!)
black shoes and black bow tie. Candidates may not wear
Knights of Columbus jewels or name badges. Jewels of
office may be worn by active officers only. Members
of the Armed Forces, policemen and firefighters may
wear their full dress uniform in lieu of tuxedo.
Priests may wear clerical garb with Collar.
All Sir Knights, especially the new Sir Knights, should attend the banquet with their ladies and guests to celebrate the Exemplification of the Fourth Degree. Dress code for the banquet will be formal wear. For Sir Knights that means Black tuxedo, with white tuxedo shirt, with standard black bowtie and black shoes. White dinner jacket is optional. All current dress code directives for Sir Knights apply.
Members of the Armed Forces, police officers and firefighters may wear their full dress uniforms in lieu of tuxedos. Ladies should wear evening dresses suitable for the occasion.
